the front cover. You cannot register the book without it.

4. Your institutional email address

ALSO, this address will be coupled with your book serial number for
accessing the online manuals. Only the current institutional email address
listed in your web page is accepted.
5. A Password. This password, your
email address, and your book serial number will become your permanent credentials for accessing online
manuals. Please choose your password carefully. Changing it or your email
address later will require that another application be submitted.

7. Name of your institution.
8. Departmental web page where you are shown as a faculty member.
